The Efficacy and Safety of Plant Oil Mixtures in the Treatment of Xerosis with Pruritus in Elderly People: Randomized double blind Controlled Trial

Background: Xerosis skin and pruritus is the most commonly found in elderly with
symptoms mild to severe recurrent pruritus, the skin looks rough, scaly, fissured or
cracked caused defects in the epidermal barrier function, increase co-morbidity as
chronic infections and ulcers that are difficult to heal can decrease the quality of life.
Virgin Coconut Oil (VCO) has been traditionally used for people in tropical region
as a moisturizer and anti-bacterial effect. In recent study that essential plant oil
mixture have effect synergistically as anti inflammatory, anti pruritus, promoting
wound healing may act promoting barrier skin and therapeutic. Objective: Efficacy
and safety of plant oil mixtures as therapeutic and maintenance moisturizer.for
Xerosis and pruritus in elderly Method:A randomized controlled trial was involved
61 elderly from nursing home and geriatric clinic in Moh. Hoesin General Hospital.
They were randomized into 2 groups: plant oil mixtures (n=30) and Virgin Coconut
oil (VCO) (n=31), applied on the volar legs twice a day for 4 weeks.Outcomes were
measured at baseline and weekly, week 2 and 4.To measure of clinical dry score
used ODSS; pruritus severity and side effect used VAS; for skin hydration used
corneometer CM825and for sebum level used sebumeter SM 810. Results:Clinical
dry score, pruritus severity of plants oil mixtures group were significantly decreased
than VCO at week 2 and 4 (p 0.000). Skin hydration and sebum level of plants oil
mixture group were significantly increased than VCO at week 2 and 4 (p
0.000).Clinical dry score, pruritus severity, skin hydration, andskin sebum levels of
plant oil mixtures were significantly better than VCO at week 4 (p 0.000). There is
no side effects on both of plant oil mixtures and VCO groups Conclusion: Plant oil
mixtures add pureCelin oil showed better efficacy as a therapeutin and maintanance
moisturizer for xerosis and pruritus in elderly. Both of plant oil mixtures and VCO
can be tolerated and safe to use. -
Effect of Leafflower (Phyllanthus niruri Linn.) treatment on kidney and uterus in sodium chloride -induced fibrotic rats
Views: 1,853Downloads: 794Background. Phyllanthus niruri (local name : meniran) extract is utilized in treating kidney and uterus disease. Effects of ethanol extract of P. niruri on kidney and uterus of 2% NaCl triggered fibrotic rats had been studied.
Aim of Study. Aim of this study to explore the effect of leafflower (Phyllanthus niruri Linn.) treatment on kidney and uterus in 2% NaCl - induced fibrotic rats.
Methods. Wistar rats were challenged with 2% NaCl in drinking water to induce fibrotic kidney and uterus. Phyllanthus niruri extract (200 mg/kg) was given orally for 4 weeks. Matrix extracellular abundance was determined by HE Staining and measured by METAVIR Fibrosis Score. The results of this study were assayed by SPSS 16.
Results. P. niruri extract was potent to reduce the matrix extracellular in 2% NaCl-induced rats compared to control group (p<0.05).
Conclusion. P. niruri extract was potent to decrease the matrix extracellular in the kidney and uterus of the rats.

The Potential of CO2 Incubator "Sriwijaya CO2 Incubator" Against Cell Culture Proliferation In Invitro Study As Smart Controlling-Based CO2 Incubator For Cell Culture

Introduction. A CO 2 incubator is an essential tool for the initiation of the
proliferation of primary culture cells or cell lines. In principle, this tool works by
keeping the sample cell line at an optimum temperature of 37 o C and 5% carbon
dioxide supply. The ability of the CO 2 incubator to maintain temperature and supply
of 5% carbon dioxide are essential points in the development of the CO 2 incubator.
This study is an attempt to convince the potential of Sriwijaya CO 2 Incubator in
maintaining the proliferation ability of cultured cells in an in vitro study. Methods.
This study is an experimental pre-post test that explores the percentage of viability
of primary culture cells (fibroblasts) before and after incubation in CO 2 incubators.
The object of this study was fibroblast cells obtained from the prepuce of patients
who performed circumcision. Results. Fibroblast cell proliferation in CO 2 incubators
shows an increase in the number of fibroblast proliferation which can be seen with
the increasing number of cells visualized by inverted microscopy. Conclusion.
Sriwijaya CO 2 incubator has the potential to be used in in vitro research to trigger
the growth and proliferation of fibroblast cells. -
Gendola Leaf Ethyl Acetate Fraction (Basella rubra Linn) Reduces Spermatozoa Motility and Viability in Vitro

Background. The use of plants as anti-fertility in men has been done to reduce side
effects. The Gendola plant is one of the Basellaceae family plants, which is a natural
medicinal plant in Indonesia. Gendola leaves contain phenol compounds, flavonoids,
tannins, steroids and triterpenoids which can have cytotoxic activity. This study
aims to determine the in vitro anti sperm activity of the ethyl acetate fraction of
Gendola leaves on male spermatozoa. Methods. This study is an experimental
laboratory in vitro study using male sperm preparations. The sample used was 20
men for each group, both the group was given the gondola leaf fraction and the
control group. The treatment group was given ethyl acetate fraction of Gendola leaves
(Basella rubra Linn.) With a concentration of 0.05% for 5, 10 and 15 minutes, the
viability and motility of sperm were calculated. Data analysis was performed using
the Mann Whitney test (p <0.05) using SPSS release for Windows software. Results.
The results showed that the ethyl acetate fraction affected the motility and viability
of spermatozoa. When compared with the control group, the motility treatment group
obtained was classified as non-progressive. In the treatment group, the viability of
spermatozoa was significantly different at minute 5, minute 10 and minuted 15 (p
<0.05). Conclusion. Ethyl acetate fraction has potential as an anti sperm in
reducing sperm motility and viability. -
Prescription Profile of Antibiotic Drugs Post Excision Biopsy of Oral Soft Tissue Disease in Palembang

small benign tumors in the soft tissues of the oral cavity. Antibiotics are prescribed
by dentists as a follow-up therapy after excisional biopsy to prevent bacterial
infection. The purpose of this study was to determine the profile of antibiotic drug
prescription after excision biopsy of oral soft tissue disease by dentists in Palembang.
Methods This type of research is descriptive observational with a survey method.
The research was carried out by distributing questionnaires to 40 dentists and
dental specialists in Palembang who had performed excision biopsy treatment for
oral soft tissue disease. Result The results showed that the antibiotic prescription
of amoxicillin was 55%, the combination of amoxicillin and clavulanic acid was
17.5%, and clindamycin was 12.5%. The cases found in this study were epulis
57.5%, mucocele 30%, and traumatic fibroma 10%. Conclusion It can be concluded
that the antibiotic most often prescribed by dentists in Palembang after excision
biopsy treatment was amoxicillin and the most common case was epulis. -
The Influence of Knowledge, Attitudes and Behavior of Cleaning Officers with the Number of Mosquito Larvae in the Water Reservoir Punti Kayu Palembang City

Information about water reservoirs as their habitat as well as human knowledge,
attitudes and behavior factors are considered important to help eradicate mosquito.
This study aims to determine the effect of knowledge, attitudes and behavior of
cleaners on the number of mosquito larvae in water reservoirs in Palembang.
Methods This research was mixed method research (quantitative and qualitative) in
the field and laboratory. The research samples were all mosquito larvae and
containers found. The research respondents were all cleaning services. Data were
collected by interviewing and filling in questionnaires, observing water reservoirs
and identifying the types of mosquito larvae in microscope. Results A total of 40
containers were found in Punti Kayu Nature Park. There were 8 containers that were
indoors (20%) and 32 containers were outdoors (80%) with 17 containers contained
larvae. Of all the containers that were positive for larvae, 6 were filled with clear
water and 11 were filled with turbid water. The number of larvae found were 358
larvae outdoors and 82 larvae indoor with a total of 440 mosquito larvae with the
genus Culex spp. and Aedes albopictus species. The index of mosquito larvae were
obtained FLI (Free Larva index)=66,67%, HI (House Index)=33,33%, CI (Container
Index)=16,67%, BI (Breteau Index)=33.33% from the 3 buildings examined. From
these results, it was obtained DF (density figure) or density level of 5. There were 40
containers that could be habitats for Culex spp. and Aedes albopictus. Conclusion
The knowledge, attitude and behavior of the cleaning services were good. The
number of mosquito larvae could be influenced by the frequency of cleaning and the
lack of knowledge of the cleaning services. -
Relationship between Interleukin 6 Serum Levels and Degree of Depression in Systemic Lupus Erythematosus Patients Internal Disease in Dr. Moh Hoesin Palembang

Systemic Lupus Erythematosus (LES) is a chronic autoimmune inflammatory disease affecting many organ systems with varied clinical manifestations, disease course and prognosis. One of the clinical manifestations that can occur is depression as much as 25%. In LES patients, there is an increase in proinflammatory cytokines, one of which is IL-6, which is often associated with contributing to the pathophysiology of depression. The purpose of this study was to determine the relationship between serum IL-6 levels and the degree of depression in LES patients at RSUP DR. Mohammad Hoesin Palembang.
Methods. Analytic observational study in the form of correlation test with cross sectional design which was conducted in the Internal Medicine room RSUP DR. Mohammad Hoesin Palembang from May 2019 to March 2020. The number of study subjects was 40 samples who were LES patients with depression who had been calculated using the BDI (Beck's Depression Inventory). All subjects were examined for serum IL-6 levels and evaluated for factors that influence it.
Results. There were 40 study subjects aged 18-56 years. The median degree of depression was 16 with a range of values from 10 to 40. There were 17 people (42.5%) with mild depression degree, 14 people (35%) moderate depression, and 9 people (22.5%) major depression. In LES activities, there were 5 people (12.5%) with light LES activities, 20 people (50%) with moderate LES activities, and 15 people (37.5%) with heavy LES activities. The mean serum IL-6 level was 11.57 pg / mL with a value range of 1.1-50pg / mL. There was a significant correlation between the degree of depression and LES activity (p = 0.004; r = 0.444) and between serum IL-6 levels and the degree of depression (p = 0.025; r = 0.169). However, there was no significant correlation between serum IL-6 levels and LES activity (p = 0.057; r = 0.304).
Conclusion. There is a relationship between serum IL-6 levels and the degree of depression in LES patients at Dr. Moh. Hoesin Palembang with a very weak correlation.

Correlation of Vitamin D Serum Levels with Muscle Mass, Muscle Strength, and Physical Performance in the Elderly Community in Mohammad Hoesin General Hospital Palembang
Views: 1,811Downloads: 797Background: Older people tend to have decreased activity and intake of macronutrients and micronutrients that are not in accordance with the body's needs. Research in 2017 on elderly men and women at the Center for Family Compensation (PUSAKA) in Central Jakarta showed that 80.2% of elderly people have vitamin D deficiency. Low serum levels of vitamin D in the body are the initial risk of decreased muscle mass and physical performance in old age. where there is a significant correlation between low serum vitamin D levels in the body with sarcopenia and mortality. The purpose of this study was to determine the correlation of vitamin D levels with muscle mass, muscle strength and physical performance in the elderly community at RSMH Palembang.
Methods: This research is a study analytic observation with a cross-sectional approach that was carried out at the Integrated Geriatric Clinic Internal Medicine RSMH Palembang from November 2019 to November 2020. A sample of 34 people aged> 60 years were examined for muscle mass, muscle strength, physical performance and serum vitamin D levels. All processing and analysis The data in this study used SPSS version 25 for Windows.
Results: From 34 subjects, it was found that there were 29 women (85.3%) and 5 men (14.7%). The mean value of muscle mass is38.76 ± 4.1 kg / m2, long runs within 6 meters 6.12± 0.9 m / sec, and hand grip strength 24.9 ± 5.2 kg. It was found that 18 (62.1%) female samples had vitamin D deficiency while 11 (37.9%) samples had vitamin D insufficiency. There was a significant correlation between muscle mass and hand grip strength with vitamin D with weak correlation strength and length of walking with vitamin D with moderate correlation strength.
Conclusion: There is a significant correlation between muscle mass and grip strength and walking time of 6 meters with vitamin D.

NFATc1 and Tumor Associated Macrophages Affect Progression of Certain Subsets of Diffuse Large B Cell Lymphoma Non-GCB Subtypes
Views: 1,277Downloads: 463Introduction: Diffuse large B cell lymphoma (DLBCL) is the most common type of non-Hodgkin lymphoma among B cell lymphomas. The interaction of tumor cells with their microenvironment (tumor microenvironment, TME) leads to progressivity of malignancy. CD163 + macrophages known as components of TME. Nuclear factor of activated T cell (NFATc1) and MYC are important transcription factors in malignant transformation and progression. Therapeutic strategies were fast developed, nevertheless, efforts to decrease DLBCL morbidity and mortality are unsatisfied, therefore,new markers for prognosis and or therapeutic options of the patients are necessary. This study was aimed to investigate NFATc1 expression in DLBCL and its TME.
Methods: Thirty-two paraffin blocks were selected then immunostained for expression of NFATc1, MYC, and CD163. Clinopathologic data i.e. ages, gender, and proliferation index Ki-67 were obtained. Data was analyzed by statistics
Result: Positive expression of CD163 and NFATc1 was among 55% and 45% of cases respectively. All DLBCL cases in this study were non-GCB subtype and more patients were under 60 years (66%). Positive expression of CD163 was higher in males (69%) and in patients under 60 years (63%). Tissues positive for both NFATc1 and CD163 was observed higher among males and patients under 60 years.
Conclusion: NFATc1 may affect development and or progression of certain subsets of DLBCL non-GCB subtype.

The Role of Serum Omega 3 Levels on Muscle Mass, Muscle Strength, and Physical Performance in the Elderly Community

Introduction. Old age is related to geriatric syndrome where there are several health
problems that often occur related to a decrease in body function and an increase in
the inflammatory process. Decrease in muscle mass, muscle strength and physical
performance will lead to a condition of sarcopenia and frailty syndrome in the elderly.
The importance of prevention so that sarcopenia does not occur in the elderly by
evaluating the factors that can cause it, one of which is controlling nutritional factors
(macro and micro nutrients), such as in research, namely omega-3 macronutrients.
The purpose of this study was to determine the correlation between serum omega-3
levels with muscle mass, muscle strength, and physical performance in the elderly
community at Moehammad Hoesin Hospital Palembang. Methods. This research is
an observational analytic study with a cross-sectional approach which was
conducted at the Integrated Geriatric Clinic Internal Medicine RSMH Palembang
from November 2019 to November 2020. A sample of 21 people aged> 60 years was
examined for muscle mass, muscle strength, physical performance and omega-
levels. 3 serums. All data processing and analysis in this study used SPSS version
25 for Windows. Results. Of the 21 subjects, it was found that women were 19
people (90.5%) and 2 men (9.5%). The mean value of muscle mass was 37.65 ± 5.7,
hand grip strength was 24.04 ± 5 kg, and walking speed was 6.72 ± 1.8 seconds per
6 meters. There were 11 subjects with low serum omega-3 levels of 485.25 ± 110.19
mg. There was a significant relationship between serum omega-3 levels and muscle
mass (p = 0.041) with moderate correlation strength (r = 0.448). Conclusion: Muscle
mass has a significant relationship with serum omega-3 levels in the elderly with
moderate strength. Meanwhile, muscle strength and physical performance did not
have a significant relationship. -
Comparison of the Effectiveness 3% Tranexamic Acid Cream Versus 4% Hydroquinone Cream for Treatment of Epidermal Type Melasma
Views: 7,893Downloads: 1,542Background: Melasma is an acquired hypermelanosis in areas that are exposed to sun. especially in face. Severity of melasma is measured using modified Melasma Area Severity Index (mMASI) and Melanin Index (MI). Hydroquinone (HQ) is an effective drug and also a gold standard in terms of melasma treatment. However. the
drawback of hydroquinone is the treatment duration which is quite long. the occurence of side effects. and also recurrence potential. Several recent studies shows that one of the anti-pigmentation agent options. the tranexamic acid (TA). is able to prevent melanocyte activation. The results of various studies found that the use of TA showed a significant improvement in MASI scores.Objective: To compare the effectivity of 3% TA cream with 4% HQ cream as an alternative therapy in epidermal type melasma treatment.
Methods: A double-blind. matched. randomized clinical trial was conducted during July 2019 until August 2019 at Dr. Mohammad Hoesin General Hospital Palembang. Subjects were divided into two groups. The first group. consists of 30 participants. get 3% TA cream; and the second group. consists of 30 participants. get 4% HQ cream. Subjects who fulfilled the inclusion criteria was included through a simple randomization. Study subjects wer given random cream pot (with label A or B on it) according to the result of simple randomization. Finally. every subjects’ melasma severity was measured using mMASI and mexameter ® .
Results: There were differences in terms of baseline. 4 th weeks and 8 th weeks after treatment mMASI scores in 3% TA and 4% HQ group. At 8 th week. the mMASI score the 3% TA group was lower than 4% HQ group. There was no differences in baseline. 4 th weeks and 8 th after treatment MI between the two groups. Melanin index for 3% TA group was lower than 4% HQ group.
Conclusion: Three-percent TA cream and 4% HQ cream is effective in decreasing mMASI score and MI in epidermal type melasma. There were differences in terms of the mMASI score during the 8th week after treatment between 3% TA and 4% HQ group.
