Potential of Sriwijaya Thermal Cycler Smart Controlling-Based as a Tool for DNA Sequence Polymerase Chain Reaction

device for examining COVID-19 is getting bigger. Many laboratories have overused
the use of thermal cyclers due to the limited availability of this tool. This study aims
to test the effectiveness of the Sriwijaya thermal cycler based on a smart controller
in conducting PCR DNA sequences. Methods. The research design was an
experimental study with a posttest control group approach, in order to see an
overview of the results of the polymerase chain reaction (PCR) in the form of
intermediate DNA bands using Sriwijaya Thermal Cycler compared to groups using
factory-made Thermal cyclers. Results. The PCR results showed precise results from
the image and band separation, and there was a clear separation of the bands on
the marker. The PCR results from the ACE I / D gene showed quite good and optimal
results in the separation of the PCR results band. Conclusion. Sriwijaya thermal
cycler is effective in DNA polymerase chain reaction (PCR) sequences comparable to
that of the manufacturer's Thermal Cycler. -
Activity of Cytochrome P450 Monooxygenase (CYPs) Metabolic Enzymes as Markers of Insecticide Resistance in Anopheles vagus Muara Enim Mosquitoes, Indonesia
Views: 1,386Downloads: 651The use of synthetic pyrethroids in insecticide-treated bed nets (LLINs) and spray form (IRS) has been carried out since 2012 in Muara Emil Village and since 2016 in all villages in Tanjung Agung District, Muara Enim Regency. Biochemical resistance can occur to the enzymes that play a role in detoxifying the insecticide. This study aims to identify whether there is an increase in the levels of the enzyme Cytochrome P450 monooxygenase (CYPs) through the enzyme biochemical test on the malaria vector mosquito Anopheles vagus originating from Pagar Dewa Village and Muara Emil Village, Muara Enim Regency. The sample of female Anopheles mosquitoes was not full. Blood was taken at night using the resting collection method. Mosquitoes that have been identified as Anopheles vagus species are used to check CYPs enzyme levels using the ELISA method. The results of the biochemical test showed that enzyme levels increased from the Muara emil and Pagar Dewa village mosquitoes with a percentage of 71% and 61.53% (with cut off point OD> 0.165). The value of CYPs enzyme levels for mosquitoes from Muara Emil village was higher than that of the mosquitoes from Pagar Dewa Village (mean 0.005027 ± 0.007). The increased activity of the CYPs enzyme plays a role in the detoxification of synthetic pyrethroid insecticides that can cause resistance. The high levels of enzymes from the villages of Muara Emil and Desa Farah Dewa are in line with intensive insecticide exposure in eradicating malaria vectors in the highest malaria transmission areas in Muara Enim Regency.

The Effect of Ethynil Estradiol and Desogestrel on Proliferation and Apoptosis Hydatidiform Mole Trophoblast Cell
Views: 1,216Downloads: 402Background: To explore the effect of ethynil estradiol and desogestrel on proliferation and apoptosis hydatidiform mole trophoblast cell.
Methods: From April 2008 until March 2009, we collected 15 samples of hydatidiform mole tissue. Trophoblast cells was isolated and culture at RPMI 20% FBS medium. Only 7 samples (46%) shown good growth. Cell was identified using cytology and β HCG test. Trophoblast cells good quality than devided into three groups observation. First group get ethynil estradiol 10 nmol/mL, second group get desogestrel 100 nmol/mL, Third group get DMSO 1%. Cells incubated and observe at 24, 48, 72, 96 hours. Cell cycle, apoptosis and β HCG was evaluated at each time observation. Cell cycle evaluation using BD cycle test plus DNA reagent, apoptosis evaluation using FITC-Annexin V. Analyze using FACSCalibur flowcytometer. β HCG evaluation using Abbott AxSym total β-HCG reagent pack.
Results: The group of cell that get ethynil estradiol in concentration 10 nmol/mL had cell proliferation index, amount cells and β HCG level higher than control after 72 hours observation. The group of cell that get desogestrel in concentration 100 nmol/mL have cell proliferation index, amount cells and β HCG level lower than control after 48 hours observation. There are no differences of apoptosis between the two group and control.
Conclusion: Ethynil estradiol will increase proliferation of hydatidiform mole trophoblast cell, while desogestrel will decrease proliferation of hydatidiform mole trophoblast cell. There are no effect of ethynil estradiol and desogestrel on apoptosis of hydatidiform mole trophoblast cell.

Analysis of Neuron Specific Enolase Serum Levels in Traumatic Brain Injury
Views: 1,283Downloads: 768Background : Traumatic brain injury is still the main cause of death and disability in productive age. Assessment the level of consciousness and imaging examinations after a brain injury can not always describe the severity of damage in the brain, this is because the pathological process is still ongoing due to secondary brain injury. Therefore, it is necessary to examine biomarkers that can describe the severity of the pathological process that occurs. The purpose of this study was to assess serum neuron-specific enolase (NSE) levels and their relationship to the severity and outcome of a traumatic brain injury.
Methods : A cross sectional design was conducted in the emergency department of DR M Djamil Hospital, Padang. There were 72 patients who met the inclusion criteria. A Glasgow Coma Scale examination was performed to assess the severity of brain injury and examination of NSE serum levels at 48 hours post- injury using ELISA technique and assess the Glasgow outcome scale (GOS) at 6 weeks post-injury. Data analysis using SPSS 22 program, the results are significance if the p value <0.05
Results : The average NSE level was higher in severe brain injuries than moderate and mild brain injuries and this difference was statistically significant (p<0.05). The NSE serum levels were higher in poor outcomes than in good outcomes and this difference was statistically significant (p<0.05).
Conclusion : High NSE serum levels in the acute phase were associated with the severity of the brain injury and poor outcome 6 weeks after the brain injury.

A Rare Case of Juvenile Xanthogranuloma in 8 Months Old Baby with Dyslipidemia
Views: 11,878Downloads: 1,664Background: Juvenile xanthogranuloma (JXG) is a non-Langerhans cell histiocytosis disease of childhood. But due to rarity of non Langerhans cell hystiosis itself, the exact prevalence of juvenile xanthogranuloma remain unknown with only a few epidemiological journal ever published. Juvenile xanthogranuloma usually wihout lipid abnormality and systemic involvement. But association between JXG and lipid abnormalities is still not well understood. We describe a patient with multiple cutaneous JXG who also developed hyperlipidemia.
Case: A case of a 8 months-old baby patient with juvenile xanthogranuloma is reported. Patient parents noticed yellowish dots on child’s face since six months ago, and it was gradually increase in size and number, and spread to trunks, upper and lower limb since 2 months ago. Patients got formula milk since 7 months ago. Patients father has uncontrolled hypercholesterolemia, and grandparents had controlled dyslipidemia. Dermatological state showed yellowish plaque and papule on the face, trunk, lower limb, and upper limb. Dermoscopy show yellowish papule with sun setting appearance and branched and linear vessel on orange yellow background. Laboratory finding showed elevated lipid serum. Foam cell and Touton giant cell is found on histopathology examination.
Discussion: The presented case demonstrates that skin lesions in patients with diagnosed JXG may have a variable clinical presentation, ranging from single to diffuse skin lesions, also present from the birth to childhood. The diagnosis requires histopathological confirmation to avoid misdiagnosis of malignant disease. Association between JXG and lipid abnormalities remain unknown, with most of the patient show normal lipid serum. Majority of patients presenting lesions limited to the skin requires only a strict dermatological observation.

Correlation of Peak Systolic Velocity Value with Wagner Score in Diabetic Foot Ulcer patients in RSUP Dr. M. Djamil Padang
Views: 783Downloads: 504Background: Diabetic foot ulcers are one of the chronic complications of diabetes mellitus (DM) in the form of wounds on the skin surface of the feet of DM patients accompanied by internal tissue damage or tissue death, either with or without infection, which is associated with the presence of neuropathy and or peripheral arterial disease in patients with DM. The World Health Organization (WHO) states that at least 422 million people suffer from DM A new alternative parameter for assessing the degree of peripheral ischemia is ankle peak systolic velocity (PSV). The rationale for PSV is based on the observation that in the ischemic limb, blood travels at a much slower rate in the distal leg arteries than in the nonischemic limb. PSV is a quick and easy test to assess the patient's vascular status and monitor the development of diabetic foot ulcers, and can assess the degree of ischemia and predict healing of diabetic foot ulcers. This study aims to determine the Correlation of Peak Systolic Velocity Value with Wagner Score in Diabetic Foot Ulcer patients in RSUP Dr. M. Djamil Padang.
Methods: This research is analytic observational with cross sectional design, the research uses primary data. This research will be conducted at the RSUP Dr. M. Djamil Padang from July 2021 to October 2021. The population in this study were diabetic foot ulcer patients who came for treatment at Dr. M. Djamil Padang. A sample of 32 patients Diabetic foot ulcer met the inclusion criteria. Univariate analysis was conducted to describe each of the variables studied. Numerical data is normally distributed, then it is presented in the form of mean ± standard deviation (SD), whereas if the numerical data is not normally distributed then it is presented in the form of median, minimum and maximum. Categorical data is presented in the form of frequency and percentage of each category. Bivariate analysis was started by using the data normality test using the Shapiro Wilk test (n<50). In testing the correlation between Peak Systolic Velocity and Wagner Score in diabetic foot ulcer patients, the Pearson correlation test is used if the data is normally distributed. Data analysis using SPSS version 22.0.
Results: more than half of the subjects (65.6%) are male. The mean age of the subjects was 60.46±10.06 years. More than half of the subjects (62.5%) had a normal body mass index. The average length of suffering from diabetes mellitus was 5.48 ± 2.26 years. Half had a history of smoking (59.4%), a small proportion of subjects had hypertension (21.9%), hypercholesterolemia (6.3%) and neuropathy (12.5%). More than half of the subjects (53.1%) had immunopathies. There is a correlation between the popliteal PSV value and the Wagner score in diabetic ulcer patients (p<0.05), with a moderate correlation strength (r=-0.463) and a negative direction. There is a correlation between the PSV dorsali pedis value and the Wagner score in diabetic ulcer patients (p<0.05), with a strong correlation strength (r=-0.720) and in a negative direction. Furthermore, there is a correlation between the posterior tibial PSV value and the Wagner score in diabetic ulcer patients (p<0.05), with a moderate correlation strength (r=-0.530) and a negative direction.
Conclusion: There is a correlation between the popliteal, tibialis posterior, dorsalis pedis PSV value and the Wagner score in diabetic foot ulcer patient

The Relationship of Breast Cancer Subtypes with the Event of Metastasis in Dr. M. Djamil Hospital Padang
Views: 946Downloads: 1,069Background: One of the high mortality rates from breast cancer is related to the incidence of metastases. It is known that >90% of deaths in breast cancer are related to the incidence of metastases and the complications that follow. Breast cancer is divided into several subtypes based on the expression of receptor genes in breast cancer tissue, namely Luminal A, Luminal B, HER 2 and Triple Negative Breast Cancer (TNBC). This study aims to determine the relationship between breast cancer subtypes and the incidence of metastases in Dr. M. Djamil Padang.
Methods: This study used a retrospective case-control study to breast cancer patients with metastatic at Dr M Djamil Hospital, Padang from 2016-2021. The research subjects were 260 breast cancer patients who met the inclusion criteria. The study subjects were divided into 130 patients as the case group with metastases and 130 patients as the control group with no metastases. To determine the relationship between breast cancer subtypes and the incidence of metastases, the chi-square test was used. If the p value <0.05, it can be concluded that it is significant. Furthermore, analysis is continued to obtain an odds ratio (OR) in identifying risk opportunities with Cochran's and Mantle-Haenszel statistics common odds ratio estimate. The data were analysed using the Statistical Package for Social Sciences (SPSS) program.
Result: Characteristics of the subjects in this study can be seen that there is a relationship between hormonal contraception, T and N status with the incidence of metastasis (p <0.05). Patients with metastases were more common with breast cancer subtypes luminal B (61.5%), HER2+ (21.5%), TNBC (14.6%) and luminal A (2.3%). The most common locations for breast cancer metastases were lung (48.5%), bone (26.2%), liver (19.2%), brain (5.4%) and other places (0.8%). There was a relationship between breast cancer subtypes and the incidence of metastasis (p<0.038). The highest risk of metastases was in patients with TNBC subtype with OR = 7.74 (95% CI 1.72-34.79). There was no relationship between breast cancer subtypes with metastatic location (p>0.05) and breast cancer subtypes TNBC had a risk (OR) of 9.60 (95% CI 1.96-47.14) times increasing the risk of metastases in brain.
Conclusion: It can be concluded that there was a relationship between breast cancer subtypes and the incidence of metastasis

Correlation between Intraoperative Ossicular Status and Conductive Hearing Loss Degree among Chronic Suppurative Otitis Media Patients in Dr Mohammad Hoesin General Hospital Palembang
Views: 1,356Downloads: 1,233Background: Chronic suppurative otitis media (CSOM) is a chronic inflammation of the mucosa and periosteum of the middle ear and mastoid cavity that defined as a perforated tympanic membrane with persistent drainage for more than 2 months. Preoperative hearing threshold and air bone gap (ABG) assessment are expected to predict the ossicular status which can only be ascertained intraoperatively. This study aimed to determine the correlation between intra-operative ossicular status and the degree of conductive hearing loss assessed based on the hearing threshold and average ABG among CSOM patients in RSUP Dr. Mohammad Hoesin Palembang.
Methods: Observational research using a cross sectional design. The data were collected using medical record on 64 subjects with a diagnosis of CSOM who underwent mastoidectomy surgery at Dr. Mohammad Hoesin Hospital Palembang for the period of March 2019 to June 2021.
Results: From 64 samples conducted in the study, the average ossicular status score in CSOM patients was 1.84 ± 1.48, with the highest group scoring 0 being the malleus incus and intact stapes as many as 21 patients (31.3%). There was a strong positive correlation between hearing threshold scores and intra-operative ossicular status scores (p<0.005, R=0.5) and there was a strong positive correlation between ABG scores and intra-operative ossicular status scores (p <0.005, R=0.6). From the linear regression test, the most influential in predicting intra-operative ossicular status scores were gender, hearing threshold value, ABG value, and the presence of cholesteatoma
Conclusions: There is a significant relationship between intra-operative ossicular status and the degree of conductive hearing loss in CSOM patients.

The Association between Urinary Tissue Inhibitor Metalloproteinase 2 (TIMP-2) and Insulin-like Growth Factor Binding Protein 7 (IGFBP-7) and Renal Recovery in Acute Kidney Injury

Acute kidney injury (AKI) is a common and serious medical condition associated with significant increases in morbidity, mortality, cost of care and non recovery of kidney function that leads to progression to chronic kidney disease. Cell cycle arrest is implicated in the pathogenesis and repair process following AKI. The urinary cell-cycle arrest markers tissue inhibitor of metalloproteinase-2 (TIMP-2) and insulin-like growth factor binding protein 7 (IGFBP-7) have been utilized to predict the risk of AKI in many studies from specific population with good performance. However, their use in predicting recovery is still lacking. The aim of this study was to determine the association between two novel AKI biomarkers, urinary TIMP2 and IGFBP7 and renal recovery after 7 days of treatment in AKI patients at Dr. Mohammad Hoesin Hospital Palembang.
Method. This was a prospective cohort study conducted in dr. Mohammad Hoesin Hospital Palembang from January 2021 until March 2021. Subjects enrolled in this study were patients whom diagnosed AKI based on KDIGO 2012 criteria. Urine samples were collected upon patients’ enrollment within 24 hours of AKI diagnosis. We utilized Sandwich Enzyme Linked Immunosorbant Assay (ELISA) method to detect urinary TIMP-2 and IGFBP-7 levels. The primary outcome is recovery from AKI after 7 days of treatment. Chi square test is used to analyze the association between urinary TIMP-2 and IGFBP-7 levels and renal recovery.
Results. There were 70 subjects, only 22 of them were recovered after 7 days (31%). Median of urinary TIMP-2 and IGFBP-7 was 0,0047(0,0001-0,1439) [(ng/ml)2/1000]. There was significant association between urinary TIMP2 and IGFBP7 and renal recovery (p=0,027; OR 3,19; 95% CI 1,116-9,128).
Conclusion. There was significant association between urinary TIMP2 and IGFBP7 and renal recovery in AKI patients.

Success Rate on Double-J Stenting on Gynecological Cancer Patients: A Meta-Analysis Study
Views: 843Downloads: 1,136Background. Metastatic cervical cancer in the ureter may lead to obstructive uropathy due to mechanical ureteral blockage. The insertion of a double-J (DJ) stent is a urinary diversion option that can be offered to the patients, but it becomes challenging due to the high rate of failures. This study aim to analyze the success rate on DJ Stent placement on patients with gynecological malignancy.
Methods. The study was a systematic review using medical journals collected from Google Scholar, BMJ and PubMed. The journals were searched from 2015 to 2020. The database search throughout 2015-2020 using “Double J Stent’, “Gynecological”, “Malignancy”, “Kidney failure”, “Hydronephrosis” and “Urethral Obstruction”. A literature search was performed and evaluated based on established criteria. The quality assessment of included studies done according to the PRISMA. Meta analysis were done using the fixed effect model or random effect model.
Results. The identified article on each med were collected using Mendeley Desktop software. There were 101 scientific publication from PubMed, 232 from Scopus, 1 from Cochrane and 60 from reseachgate results found using the combined keywords. The total of 7 publications matched all the inclusion criteria in this study.
Conclusions. Patient’s general well-being could be measured by the staging of the malignancies and kidney function laboratory test. Another factor should be think through was the side effects of the chemotherapy or radiotherapy, as well as the compliance of the patient during stent maintenance.
