A Study Demographic, Histopathological Patterns and Clinical Profile of Oral Squamous Cell Carcinoma in East Java, Indonesia: A Hospital-Based Study
Views: 1,079Downloads: 478Background. Cancer remains a health problem in the world. Estimated 12,7 million new case and 7,6 deaths cause of cancer each year. Oral carcinoma is one of head and neck carcinoma. Oral carcinoma is 6th the most cancer in the world and 90% of the cases are squamous cell carcinoma. The purpose of this research is to study the clinic-epidemiological profile among Indonesian patients associated with oral cancer cases managed in Head and Neck Surgery Unit of Dr Soetomo General Hospital between January 2013- December 2016.
Methods. Records of 116 cases were reviewed for clinical details, histopathological data, and follow-up status. Study variables included demographic factor, modifiable risk factors such as tobacco usage and alcohol consumption, site involved, staging, histopathological result, and treatment modalities. Data was presented in form of percentages and proportions.
Results. Out of the total 116 cases, majority of the subjects were above 40 years old. Twenty-two (22%) of subjects were young adult (below 40 years old). Seventy-seven subjects (77 %) were male. It was observed that 16 cases (21%) of male subjects was a smoker before they had disease. No female subject consumed tobacco in all form or drinking alcohol. The most common site involved was tongue, followed by lower ginggivo-buccal complex. Histopathologically, 110 cases (95%) was well-differentiated squamous cell carcinoma. Most of the cases (88.8 %) were in advanced stage (stage III or IV) and were treated with surgery followed by adjuvant therapy.
Conclusions. The most common site for oral cancer was tongue and majority of the cases were well-differentiated squamous cancer presented in advanced stage of disease. We observed that alcohol consumption and smoking did not have any role in causing oral cancer in women, therefore further study needed to evaluate other possible factors such as diet pattern and oral hygiene. Cigarette smoking might be the cause of oral cancer in male patients. -
The Safety of Cardioversion in Supra Ventricular Tachycardia Patient with Pregnancy
Views: 1,080Downloads: 793Backgrounds. Supraventricular tachycardia is a type of tachyarrhythmia characterized by sudden changes in heart rate and increases rapidly. In supraventricular tachycardia, the abnormalities that occur include components of the conduction system and occur in the upper part of the HIS bundle. This case report describes a case of supraventricular tachycardia suspected of AVNRT in a pregnant woman accompanied by hypokalemia, hypocalcemia and prolonged QT Interval along with cardioversion which was performed as the management of this case.
Case presentation. A woman, Mrs Y, 24 years old, a housewife, having her address within the city of Palembang. The patient came to the emergency department of Moh. Hoesin Hospital Palembang with the chief complaint of chest palpitations. On physical examination, there was a grade 2/6 murmur, no shortness of breath, the patient was 7 months pregnant. He had a history of heart disease SVT 8 years ago. ECG examination showed sinus rhythm with low atrial rhythm in leads II, III, AVF. On laboratory examination, the blood calcium level was 8.3 mg/dL. Management of this patient includes pharmacological and non-pharmacological management. On the 14th day of hospitalization, 100 joules of cardioversion was performed and the patient responded well. The patient's condition improved on the 20th day of treatment.
Conclusion. The patient experienced a good and effective response to 100 joules of cardioversion which was performed on the 14th day of treatment. Maintenance treatment in the form of drugs in the form of diltiazem due to the condition of the patient who is 27 weeks pregnant. The next management is planning the birth process according to the patient's hemodynamics and preventing the occurrence of SVT in subsequent pregnancies. -
Factors Affecting Delivery Methods in Severe Preeclampsia Patients at Dr. RSUP. Mohammad Hoesin Palembang January 2018 – December 2019
Views: 1,458Downloads: 1,752Background: Hypertension in pregnancy (HIP) is the second highest cause of maternal death in Indonesia. Termination of pregnancy in patients with HIP or preeclampsia is the most common treatment to save both the mother and the fetus. There are two methods of delivery that can be done, namely vaginal or abdominal termination (sectio caesaria). Due to the high incidence of severe preeclampsia in Indonesia and the various determinants in choosing a delivery method for severe preeclampsia, the aim of this study is to analyze what factors influenced it in Dr. Mohammad Hoesin Palembang Hospital.
Method: This type of study is an analytic observational study with a cross-sectional study design. A total of 210 samples were taken using the simple random sampling technique from the medical records of severe preeclampsia patients who gave birth at Dr. Mohammad Hoesin Palembang Hospital during January 2018 – December 2019 and was analyzed by univariate, bivariate and multivariate analysis with SPSS ver.26.
Result: A total of 81 (38.6%) PEB patients underwent vaginal delivery and 129 (61.4%) underwent a caesarean section delivery. From the observations, 67 people (57.3%) were aged 20-34 years, 40 people (78.4%) with a gestational age of 34-36 weeks, 92 people (57.9%) multiparity, 104 people (62, 3%) without a history of hypertension, 86 people (53.4%) without a history of SC, 98 people (84,5%) with a bishop score ≤5, 116 people (59,5%) without PJT, 122 people (61%) without macrosomic fetuses, 108 people (63.5%) without PROM, and 84 people (75%) with complications underwent cesarean delivery. The factors of gestational age (p value = 0.001), history of sectio caesaria (p value = 0.000), bishop score (p value = 0.000), and complications or comorbidities (p value = 0.000) had a significant relationship to the method of delivery for patients with PEB with factors bishop score is the most influencing factor (OR = 10,299).
Conclusion: Therefore, gestational age, history of CS, bishop score, and complications or comorbidities are factors that influence the delivery method of PEB patients.

Congenital Heart Diseases in Pregnancy
Views: 1,291Downloads: 1,779This research aims to shed light into congenital heart diseases, the pathophysiology, and the ultrasonographic findings of congenital heart diseases. Congenital heart diseases are a major health concern, affecting 1.35 million children born every year. Ventricular septal defect, atrial septal defect, and atrioventricular septal defect are found in 57.9% cases of congenital heart diseases. The risk factors include consanguineous marriage, family history of congenital heart diseases, old maternal and paternal age, and exposure to teratogens, and genetic factors. Missteps in cardiac development are the main pathophysiology of congenital heart diseases. Ultrasonography screening in 18–22 weeks gestational age is utilized to screen. Follow-up screening can increase detection rate to 80%. This study has limitation of only discussing most common congenital heart diseases and did not delve into rarer types of congenital heart diseases and did not discuss impacts or burden of congenital heart diseases in adulthood and health comorbidities associated. This literature review is beneficial for general practitioners and obstetricians focusing in maternal fetal medicine.

Relationship of Food Leftover with Covid-19 Inpatients Recovery Speed in Palembang Indonesia
Views: 1,464Downloads: 613Background : Hospital nutrition services support the patient's recovery process, which the activity is feeding. One indicator of quality in food service is food leftover. The high food waste can affect the patient's health. The high food waste will pose a risk of malnutrition in hospitalized patients. The study aimed to analyze the food leftovers in COVID-19 inpatients at the Palembang referral hospital.
Methods : This cross-sectional quantitative descriptive study was with a sample of 112 patients selected by purposive sampling in February-May 2021. Data were taken using the Comstock form and then translated in the form of percent leftover food. According to the Minister of Health, the minimum leftover food is less than 25 percent.
Results : The results showed that more than 25 percent of the leftover food were 51 respondents; Thirty-three respondents (54.1%) have fully recovered and 28 respondents (45.9%) still in improvement. On the other hand, the leftovers food of less than 25 percent were 51 respondents; Forty respondents (78.4%) have fully cured, and 11 people (21.6%) still in treatments. The analysis of Chi-square test results shows a p-value of 0.013, showing a significant relationship between food waste and the recovery of COVID-19 patients.
Conclusion : The food waste of COVID-19 patients in referral hospitals was still relatively high. It is necessary to modify the diet menu to prevent leftovers and speed up the patient's recovery. -
Anatomical and Hemodynamic Evaluation of Mitral Stenosis Patients with Echocardiography
Views: 1,115Downloads: 2,371Mitral stenosis (MS) is the most common valvular heart disease encountered in developing countries. The cause of MS is almost always chronic rheumatic heart disease. Echocardiography is the single most important diagnostic tool in the evaluation of MS. The objectives are to confirm the etiology, to assess the severity of stenosis, to recommend the type and timing of intervention, to assess other valvular lesions, presence of thrombus, and vegetation. According to current guidelines and recommendations for clinical practice, the severity of MS should not be defined by a single value but rather be assessed by a multimodality approach that determines valve areas, mean Doppler gradients, and pulmonary arterial pressures. The European Society of Echocardiography/American Society of Echocardiography (EAE/ASE) recommendations of measurement method for clinical practice were categorized into three level of recommendations. Mitral valve area (MVA) can be assessed by planimetry using either 2D or 3D imaging, pressure half-time (PTH), the continuity equation, and the proximal isovelocity surface area (PISA) method. These result echocardiographic examinations can increase the accuracy and appropriate management with a good prognosis.

Role of Fibroblast Growth Factor-23 in Coronary Slow Flow Phenomenon Pathogenesis
Views: 858Downloads: 440The phenomenon of angina chest pain without significant epicardial coronary artery stenosis, but accompanied by a slowdown in coronary blood flow is often found in patients with symptoms of acute coronary syndrome who undergoing invasive coronary angiography. This phenomenon of slow coronary blood flow is then called the coronary slow flow phenomenon (CSFP). The pathogenesis mechanism of CSFP remains unclear. The pathogenesis of CSFP is thought to be multifactorial. Endothelial dysfunction, small vessel disease, inflammation, renin system angiotensin aldosterone, atherosclerosis are thought to be involved in the pathogenesis of CSFP. Cardiovascular disease incidence and death were associated with elevated levels of Fibroblast growth factor-23 (FGF-23). High levels of FGF-23 can lead to formation of blood vessel calcification, left ventricular hypertrophy, arterial stiffness, endothelial dysfunction, increased inflammatory markers and elevated levels of angiotensin II. It is suspected that FGF-23 has a role in this event other than as a regulator of bone and mineral metabolism. This literature review aims to determine the relationship between fibroblast growth factor-23 and the pathophysiology of CSFP. Based on the broad role of FGF-23, it is possible that FGF-23 is involved in the pathogenesis of CSFP.

The Correlation of No Footwear Use and Soil Helminth Incidence among Elementary School Children in Musi Rawas, South Sumatera, Indonesia
Views: 1,502Downloads: 1,762Background: Soil-transmitted helminth infection (STH) is a parasite infection that involves humans being infected with roundworms by route of soil contamination. One billion individuals are infected with worms, including 568 million school-age children. Helminthiasis in elementary school-aged children was not documented in Musi Rawas Regency. This study's goal was to identify if not wearing footwear increases the incidence of parasitic infection.
Methods: The research was a cross-sectional survey, followed by statistical analysis. The study involved elementary school-aged students in Tuah Negeri District, Musi Rawas Regency, in 2021 and at least 200 participants. This study sample consisted of 108 with a purposive sampling method. This study utilized questionnaires and stool examinations using the Kato Katz method. Chi-square and multivariate logistic regression were used for statistical analysis.
Results: Positive helminth infections amounted to 37,1% of the total (n=108). STH was comprised of 17.6% Ascaris lumbricoides, 9.3% Trichuris trichiura, and 25.9% hookworms. The finding of this research demonstrated a substantial (p = 0.000) relationship between the use of footwear and the advent of parasites. The logistic regression analysis results revealed that the most critical variable influencing the incidence of helminthiasis was not wearing any footwear.
Conclusions: The study's findings suggest a correlation between footwear use and the risk of worm infection; as a result, it was recommended that children be thoroughly educated on personal hygiene, specifically footwear use, when using the bathroom. -
Comparison of Omega-3 Serum Level Between Hiv-Aids Patients Along with Their Clinical Profiles and Healthy Population in Mohammad Hoesin General Hospital Palembang

It causes reduction in body immune system and inflammation that can affect its clinical profile. Undergoing ARV therapy patients causes intestine microbiota dysbiosis and translocation that lead to imperfect absorbtion of fatty acid. One way to control microbiota dysbiosis is by controlling the nutritional factor, especially with omega-3. However, different regions upstands different custom in fish consumption especially those of which are rich in omega-3. The purpose of this research is to compare omega-3 serum level between HIV-AIDS patients and healthy population, as well as analyzing the correlation of omega-3 serum level with of HIV-AIDS patient clinical profile.
Methods. The research will be applying observational analytic study using comparative study approach which will be performed in Tropical Infection Internal Medicine polyclinic and Medical Check-Up Polyclinic in RSMH Palembang starting from April 2020 until January 2021. Sample consisted of 32 HIV-AIDS patients from 20-58 years of age and 16 healthy population from 27-35 years of age on whom will be performed physical examination as well as clinical profile and omega-3 serum level examination. All data processing and analysis will be performed using SPSS v.25 for windows.
Results. Out of 32 HIV-AIDS patients, we obtained low omega-3 serum level with median value at 3 (2-4) Umol/L, while in healthy population was 4 (3-6) Umol/L. Multivariate analysis inferred that the lower omega-3 serum level correlated with HIV-AIDS stage, duration of ARV therapy, blood pressure, and sex.
Conclusion. Omega-3 serum levels in HIV-AIDS patients were lower than the healthy population. There was a correlation between omega-3 serum levels and their clinical profile of HIV-AIDS patients in Mohammad Hoesin General Hospital Palembang. -
Comparison of Interleukin-1β Levels In Open and Closed Fracture Patients of The Long Bone In Padang

The purpose of this study was to determine the difference in levels of interleukin-1β in open and closed fractures of long bones in Padang
Method. This type of research is analytic observational with cross sectional design with primary data. The data were taken from the examination of IL-1β levels in long bone fracture patients at RSUP Dr. M. Djamil, RSKB Ropanasuro and RST Reksodiwiryo Padang for 4 months starting from March 2021 to July 2021. The sampling technique in this study used non-probability sampling with the consecutive sampling method. The analysis was carried out using the Independent sample T-Test to see the difference in mean IL-1 which would be calculated for open fractures and closed fractures of long bones.
Results. The results showed that most (92.9%) of the patients were male, 50% had open fractures, and 50% closed fractures. The mean value of the patient's age was 34 years and the mean level of IL-1β was 555,951 pq/L. Most of the fracture sites in patients were tibia and fibula (35.7%) and femur (35.7%) and 28.6% were radius and ulna. The results showed that there was a significant difference in the mean levels of IL-1β in patients with open and closed fractures of long bones (p-value = 0.007)
Conclusion. There are differences in the levels of interleukin-1β in open and closed fractures of long bones in Padang. This research is expected to provide consideration to support examinations in health services, in this case in fracture patients -
Coronary Microvascular Dysfunction
Views: 708Downloads: 462Coronary microvascular dysfunction (DMK) is a condition of patients who are accompanied by complaints of chest pain where the results of coronary angiography examination are normal and this is almost 49% with 60% of patients diagnosed with DMK. Another study said that about 40% of patients with DMK showed coronary flow reserve (CFR <2) of about 40% and the WISE study (Women's Ischaemia Syndrome Evaluation) showed that about 47% of patients with chest pain had normal coronary arteries. DMK can be divided into 4 groups; DMK with no coronary arterial disease (CAD) obstruction and myocardial disease, DMK with myocardial disease where this occurs due to remodeling of intramural coronary arteries, DMK with CAD (coronary arterial disease) or acute myocardial infarction with or without ST segment, iatrogenic typhoid DMK occurs after coronary recanalization caused by vasoconstriction and distal embolization. The mechanism of action of DMK can be caused by several factors, namely endothelial dysfunction, smooth muscle dysfunction, decreased diastolic perfusion time, damage to blood vessels and damage to the vascular and microvascular remodeling. And to enforce this DMK, there are several tests carried out in diagnosing the disease, some of which are invasive and non-invasive so that by enforcing the diagnosis of this disease, treatment for DMK can be done immediately and optimally.

The Relationship between Vitamin D and Autism Spectrum Disorder
Views: 855Downloads: 741The tendency for autism spectrum disorders or also known as Autism Spectrum Disorder (ASD) is increasing globally, even becoming 1 per 54 in 2016. Until now, the cause of autism is not known for certain, ASD is considered to be a combination of genetic and environmental factors. One of the environmental factors in this case is related to nutrition, one of which is vitamin D. Vitamin D deficiency is often found among children with ASD. Several studies have shown that vitamin D is involved in various brain bioprocesses including neuromodulation and nerve transmission and brain function while also influencing inflammatory processes, autoimmune disorders, oxidative stress and also neurotransmitters that are widely associated with the possibility of ASD. This review aims to describe vitamin D deficiency may contribute to ASD disorders. Based on this, in the future, it is necessary to consider when treating patients with ASD to consider the need to check the patient's vitamin D levels and if there is a deficiency it can be advised to sunbathe and or be given additional vitamin D intake.
