SARS-CoV-2 Serology Antibody in Children with MIS-C (Multiple Inflammatory Syndrome in Children) Suspected
Views: 891Downloads: 538Background: Multiple inflammatory syndromes in children (MIS-C) relate to COVID-19 severity in children. SARS-CoV-2 serology antibody is one of the diagnostic tools of MIS-C. The study aimed to describe the yield of serology antibodies of MIS-C and some characteristics found in hospitalized children with MIS-C suspects.
Methods: This research was an analytic observational study. The data were collected retrospectively from some children who were hospitalized in Dr. M. Djamil General Hospital, Padang, West Sumatra, Indonesia, from April - June 2021. The inclusion criteria were children aged 1 month to 18 years, with or without contact history with the person who probable or confirmed COVID-19 and have signs and symptoms as MIS-C base on WHO criteria.
Results: About eight out of 44 children showed positive serology antibodies and were diagnosed as MISC (18,2%). Based on demographic characteristics, children aged 11-15 years (27.3%) and boys were more affected (52.3%) as MIS-C suspected. Most of them were referred from a 2nd-level hospital outside Padang City (70.4%), but only 13.6% had a history of contact with COVID-19 confirmation patients. There was a significant difference in cardiovascular signs and symptoms between positive and negative serology antibody SARS-CoV-2 among children with MIS-C suspected (p<0.05), but not in fever, respiratory distress, gastrointestinal, neurology symptoms, either in laboratory results such as leukocytes, CRP and D-dimmer. Intravenous immunoglobulin, steroid, and PICU admitted showed no significant differences between the 2 groups, but more death prevalence in positive than negative. However, no significant differences (12.5% vs. 11.1%, p>0.05).
Conclusion: Cardiovascular signs and symptoms could be proposed to be one of the significant differences in clinical conditions to differentiated children with MIS-C suspected and MIS-C due to serology antibody results.

Diagnosis and Treatment-Spectrum Beta-Lactamase and Multidrug Resistance Bacterial
Views: 664Downloads: 704Multidrug-resistant (MDR) is a condition resistant to at least one type of antibiotic from 3 classes of antibiotics. Extended-Spectrum Beta Lactamases are globular proteins that consist of alpha-helices and beta-pleated sheets. β-lactamase hydrolyze broad-spectrum cephalosporin with oxyimino side chain. ESBL hydrolyze antibiotics group penicillin, cephalosporin first, second, third, fourth generation, and monobactam aztreonam. Multidrug resistance occurs through two mechanisms, bacteria accumulate multiple genes encoding resistance to one antibiotic, and due to increased expression of genes encoding multidrug effluent pump, enzymatic inactivation and target structure change. Multidrug-resistant (MDR) caused by extended-spectrum resistance beta-lactamase (ESBL) can be detected by phenotyping and genotyping methods. Treatment for MDR ESBLs other than carbapenems can be β-lactam/β-lactamase inhibitor combinations (BLBLIs), namely piperacillin/tazobactam (PZT).

Anxiety Related to Insomnia Severity Level in Medical Pre-clinic Students of the Faculty of Medicine, Public Health, and Nursing Science (FK-KMK), Universitas Gadjah Mada
Views: 835Downloads: 542Background: Anxiety can cause insomnia or sleep disturbances. Sleep disorders are characterized by abnormal sleep patterns that interfere with daily activities. This study aims to describe the relationship between the incidence rate and the severity of insomnia in medical students in the preclinical phase of the Faculty of Medicine, Public Health, and Nursing, Universitas Gadjah Mada.
Methods: This research is an observational study with a cross-sectional design. This research was conducted online and carried out from October 2020 to June 2021. Data collection was carried out for 2 weeks using Jotform as a data retrieval medium. The questionnaire used is generalized anxiety disorder-7 (GAD-7) to measure the level of anxiety and insomnia severity index to evaluate the severity of insomnia. Spearman's Rank analysis test was conducted to determine the correlation between anxiety levels and the severity of insomnia.
Results: A total of 130 respondents participated in this study. The majority of respondents are women (71.53%) and are more than 20 years old. Based on Spearman's rank analysis, there was a relationship between anxiety level and insomnia severity (ρ = 0.394), and it was statistically significant with a p-value = 0.000 (p<0.05).
Conclusion: There is a relationship between the level of anxiety and the severity of insomnia in medical students in the preclinical phase of FK-KMK Universitas Gadjah Mada.

Relationship between the Number of CD4+ and CD8+ Cells in Patients with COVID-19 (Coronavirus Disease 2019) at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia
Views: 948Downloads: 388Background: The pathogenesis of COVID-19 involves complex immunological processes that can predispose to disease severity. In COVID-19 patients, there will be changes in the number of CD4 + and CD8 + cells which are part of lymphocyte cells in the specific immune system. This study aims to see the relationship between CD4+ and CD8+ cell counts in COVID- 19 patients at Dr. Mohammad Hoesin Hospital Palembang, Indonesia.
Methods: This research is an observational study with a case series approach. A total of 30 study subjects who were COVID-19 patients participated in this study. The number of CD4+ and CD8+ cells was assessed, then correlation analysis was performed with the Spearman test, p<0.05.
Results: The correlation between CD4+ and CD8+ has a value of R = 0.875 and p<0.01. This shows that CD4+ and CD8+ have a strong positive correlation and are statistically significant.
Conclusion: The increase in the number of CD4+ cells will be followed by an increase in the number of CD8+ cells in COVID-19 patients at Dr. Mohammad Hoesin General Hospital Palembang, Indonesia.

Effect of Phenytoin and Vitamin C on Granulation Tissue Thickness and Total Lymphocyte Infiltration in Enterocutaneous Fistula: In Vivo Study
Views: 797Downloads: 494Background: Enterocutaneous fistula (ECF) is still a problem in the surgical field because it causes high morbidity and mortality. The rate of closure of the ECF without surgical intervention in the evolving era of wound care and with parenteral nutrition has been reported to vary, with most studies showing closure rates in the 20 to 30% range. The administration of phenytoin and vitamin C is expected to be one of the modalities in the non-operative treatment of enterocutaneous fistulas. This study aims to assess the effect of phenytoin and vitamin C on granulation tissue thickness and the amount of lymphocyte infiltration in enterocutaneous fistulas.
Methods: This study is an experimental study with a post-test only with a control group design. A total of 24 rats were divided into control groups, P1 (phenytoin+ vitamin c), P2 (phenytoin), and P3 (vitamin c). Histopathological assessment of granulation tissue thickness and lymphocyte infiltration was performed. Data analysis was performed using SPSS with univariate and bivariate methods.
Results: The P1 group showed the thickest average thickness of granulation tissue compared to all groups. The P1 group showed a lower number of lymphocyte infiltration cells than all groups.
Conclusion: The administration of phenytoin and vitamin C was effective in increasing the thickness of granulation tissue and reducing lymphocyte cells in enterocutaneous fistulas.

Effects of Ozonized Aloe Vera Oil on Full-Thickness Excision Wound Healing: In Vivo Study
Views: 768Downloads: 490Background. Disruption to the healing process of these wounds will consume large resources and will often require long-term medical management. Aloe vera oil is rich in anthraquinone compounds, especially emodin and chrysophanol. Anthraquinones have potent anti-inflammatory effects, which have the potential to activate various growth factors and chemokines and initiate angiogenesis processes that play a major role in wound healing. The ability of ozone to trigger mild oxidative stress plays an important role in triggering a cascade of cytokines and chemokines, including the initiation of vascular endothelial growth factor (VEGF) proteins that play a role in the initiation of angiogenesis.
Methods: This study is an in vivo experimental study. A total of 60 rats were divided into 10 treatment and control groups. The treatment group was given ozonized aloe vera oil 600 mg/mL, 1200 mg/mL and 1800 mg/mL. Furthermore, the assessment of VEGF and new blood vessel formation was carried out. Data analysis was performed using SPSS version 25 software with univariate and bivariate tests.
Results: The administration of ozonized aloe vera oil was able to increase the expression of VEGF, and the number of new blood vessels in the excision wound tissue.
Conclusion: Ozonized aloe vera oil is effective in promoting excision wound tissue repair in vivo.

The Relationship between CC Ligand 5 Plasma Levels and Modic Changes in Low Back Pain Patients’ Severity
Views: 537Downloads: 381Background: Low back pain (LBP) is one of the global health problems. The most common cause of LBP is disc degeneration. Modic changes (MC) are the most common MRI features in patients with disc degeneration. As a result of cartilage and disc tissue damage, CCL 5 is released and modulates pain responses in the dorsal root ganglion. This study aims to determine the relationship between CCL5 levels and the description of Modic changes on MRI with pain levels in patients with low back pain.
Methods: This study is a cross-sectional study. The research subjects were LBP patients who were treated at the Neurology Polyclinic, Dr. M. Djamil General Hospital, Padang, in the range of March 2021-December 2021, who meets the inclusion and exclusion criteria. Subjects will be taken blood to assess CCL5 plasma levels, pain assessment using the McGill Pain Questionnaire, and undergo an MRI examination.
Results: A total of 52 subjects consisted of 23 men and 29 women with an age range of 34-77 years. Most of the Modic changes were found in the type 2 group. Based on the pain scale, there were two groups with mild pain and moderate pain. On examination of plasma CCL 5 levels, the median value of plasma CCL5 levels was 303,271 ng/L. There was a significant relationship between plasma CCL5 levels and pain levels in patients with low back pain (p = 0.004). There was no significant relationship between Modic changes and the severity of pain.
Conclusion: Plasma CCL5 levels are associated with pain levels in patients with low back pain, while Modic changes are not associated with pain severity.

Antibacterial Activity of Moringa Plants (Moringa oleifera Lam.) to Overcome Antibiotic Resistance: A Systematic Review

Moringa plant (Moringa oleifera Lam.) has antibacterial activity proven to inhibit and kill pathogenic bacteria and Multi-Drug Resistance (MDR)article review aims to collect data related to testing the antibacterial activity of Moringa plants as an effort to overcome antibiotic resistance. The method follows the PRISMA-2020 protocol by searching data on PubMed and ScienceDirect media using the keywords: "Antibacterial activity" and "Moringa oleifera". The data selection uses the Rayyan application by considering the inclusion and exclusion criteria that have been set. The final results obtained are 95 data which are experimental journals of the Randomized Control Trial (RCT) type published in 1981-2021. Moringa plants began to be investigated for their activity against MDR bacteria in 2012 and became a sign that research was already in the direction of finding solutions. Antibacterial testing was carried out in vitro (10 studies) and in vivo (1 study). The leaves have been thoroughly tested for their activity against Methicillin-Resistant Staphylococcus aureus (MRSA). The results of in vitro study obtained the range of zone of inhibition, minimum inhibitory concentration, and minimum kill concentration, respectively: 8.3-12 mm; 0.25-20 mg/ml; 0.50-20 mg/ml study in vivo showed that there was the healing of MRSA infection wounds in test animals.

Effects of Pancreatic Omentoplasty on Diabetes Mellitus with Obesity after Sleeve Gastrectomy: An In Vivo Study
Views: 643Downloads: 422Background: DM, especially type 2 diabetes mellitus (DMT2), is closely related to obesity. Insulin resistance and inflammation that occur in obese patients can be treated with SG procedures. Post-SG weight loss can reduce the lipo apoptotic process and can improve the function of pancreas β cells. The omentum is rich in potential mesenchymal stem cells and is rich in anti-inflammatory cytokines, and various growth factors, so the omentoplasty procedure is believed to be able to provide a regenerating effect on the pancreatic tissue so that the performance of the pancreas can be optimal.
Methods: An experimental study with 18 rats which were divided into control and treatment groups. The treatments were sleeve gastrectomy and pancreatic omentoplasty. Furthermore, analysis of body weight and levels of IL-10 and HOMA IR was carried out with the help of SPSS software version 25 univariate and bivariate.
Results: There was a difference in mean body weight, levels of IL-10, and HOMA IR before and after treatment, p<0.001.
Conclusion: Omentoplasty is effective in improving the condition of diabetes mellitus with obesity after sleeve gastrectomy surgery in vivo.

Psychological Stress Induced Recurrent Genital Herpes: A Case Report
Views: 2,120Downloads: 1,207Background: Herpes simplex virus type 2 (HSV-2) infection can be caused by genital herpes with a high recurrence rate. Recurrent genital herpes affected patients’ quality of life. Precise management is needed to prevent the progression of the disease.
Case presentation: This study reported a 34 years old male with recurrent genital herpes. The patient developed painful vesicles in the genital area, which turned into an ulcer. History of genital herpes and gonorrhoea urethritis were reported and had been treated. The patient had a history of having unprotected sexual intercourse with multiple partners and reported having psychological stress within 1 month. DLQI score was 12, and the DASS score was 5-85, which confirmed that the patient had anxiety. Vesicles and erosion were found in the genital area. Tzanck test showed multinucleated giant cells with borderline anti-HSV-2 IgG serologies in 2 occasional evaluations. Acyclovir 3x400 mg for 5 days was used as therapy, and no lesion was found at follow-up.
Conclusion: Psychological stress was found to be a risk factor for recurrent genital herpes

Type 1 Diabetes Mellitus Comorbid with Malnutrition in Siblings
Views: 812Downloads: 890Background: Diabetes mellitus is a disorder of the metabolic homeostasis controlled by insulin, resulting in abnormalities of carbohydrate and lipid metabolism. Type 1 diabetes mellitus is one of the most common chronic conditions in children. This study aims to describe two cases of type 1 diabetes mellitus accompanied by malnutrition in children at Dr. M. Djamil General Hospital Padang.
Case presentation: The case series describes two cases of diabetes mellitus type one with comorbid undernutrition. Both patients had a family history of diabetes mellitus. And their sibling died because of diabetes type 1. Grandmother and grandfather from the father's side had diabetes. The management of this disease consists of the management of type 1 diabetes mellitus and its complications and accompanying malnutrition.
Conclusion: The management of type 1 diabetes mellitus includes the administration of insulin according to the monitoring of blood sugar levels and the management of comorbid diseases that accompany it. Education about the use of insulin and diabetes diet patterns is very important for parents and children with type 1 diabetes mellitus.

The Role of the Managerial Function of Supervisor Model 4S towards Patient Safety Culture at Sawahlunto General Hospital

Maintaining a patient safety culture is the responsibility of every healthcare professional. This study aims to explore the relationship between the supervisory function of the 4S model and the culture of patient safety at Sawahlunto General Hospital.
Methods: This study is an observational study. A total of 52 respondents participated in this study. This study uses a questionnaire instrument to assess the role of supervisory and managerial functions in the 4S model with patient safety culture. Data analysis was performed by SEM-PLS.
Results: There is a significant relationship between the characteristics of nurses and patient safety culture at Sawahlunto General Hospital. There is a significant relationship between the characteristics of supervisors and interpersonal supervisors with the managerial function of supervisors at Sawahlunto General Hospital.
Conclusion: The managerial function of the Model 4S supervisor is related to the patient safety culture at Sawahlunto General Hospital.

Expression of Estrogen Receptor and Progesterone Receptor in Meningioma Patients

This study aimed to assess the relationship between estrogen receptor (ER) expression and progesterone receptor (PR) expression in meningioma conditions.
Methods: Analytical observational research with a cross-sectional study design. The number of samples is 25 meningioma patients who performed ER and PR examinations. The research was performed at Dr. Kariadi General Hospital Semarang. Statistical test using Spearman test.
Results: Relationship of ER and PR to degree of meningioma is p = 0.181 (r = -0,276) and p = 0,056 (-0,387).
Conclusion: There is no correlation between estrogen receptor (ER) and progesterone (PR) receptor to the degree of meningioma

Intrahepatic Cholestasis due to Cytomegalovirus Infection and Extrahepatic Cholestasis due to Sludge Bile with Confirmed COVID-19
Views: 877Downloads: 1,553Background: Human Cytomegalovirus (CMV) is a Beta-Herpes virus that leads to congenital infection in 0.4% to 2,3 % of all newborns. Diagnosis of CMV infection includes the culture of the nasopharynx, saliva, and urine. Serologic tests are also beneficial for CMV diagnosis. IgM CMV-specific antibodies can be monitored. Biliary sludge is a mixture of particulate solids that have precipitated from bile. Laparoscopic cholecystectomy offers the most definitive therapy for biliary sludge.
Case presentation: A 2-month-old boy complained yellow appearance since the age of 1 week ago. Looks pale. Defecation was sometimes colored putty since the age of 2 weeks. Blood laboratory examination with hemoglobin 8,7 gr/dL, Prothrombin time, activated partial thromboplastin time, and liver function were increased. Bilirubin, alkaline phosphatase, and Gamma GT were increased. Two-phase and abdominal USG were biliary atresia. PCR CMV examination with urine and blood sample was positive. Head USG was consistent with CMV infection. Cholangiography showed sludge bile with liver cirrhosis. Therapy gansiklovir IV for 14 days then gansiklovir per oral for 6 months.
Conclusion: Cholangiography is the definitive diagnosis for biliary sludge. CMV is a congenital infection that causes asymptomatic to multiple severe symptoms, so it is necessary to monitor children for adherence to treatment, improve nutritional status and reduce morbidity and mortality rates.

Recent Evidence on Acute Kidney Injury in COVID-19 Patients: A Narrative Review
Views: 708Downloads: 410The pandemic of coronavirus disease 2019 (COVID-19) caused by the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). The clinical manifestation of COVID-19 is broad, ranging from an asymptomatic carrier state to severe disease leading to the death penalty. There is also emerging evidence that kidneys are affected early in COVID-19. Proteinuria and haematuria have been reported in 44% and 26.7% on admission, respectively. This literature review shows clinical manifestations of acute kidney injury (AKI) in a patient with COVID-19 infection. Literature reviews are carried out on various sources found on Google Scholar and Pubmed to search for articles, journal research, case reports, systematic reviews, meta-analyses, and textbooks. Various studies demonstrate the possibility of coronavirus infecting the kidney with several mechanisms such as cytokine storm syndrome (CSS), direct viral infection, and imbalance of renin-angiotensin-aldosterone (RAAS). Haematuria and proteinuria are associated with higher mortality and may signify aggressive disease early. Thus all patients should have a baseline urinalysis. There is a number of different causes of AKI in COVID-19, and some mechanisms by which COVID-19 affects kidneys remain unclear.
